We reviewed evidence on the effects of topical and systemic treatments for hand eczema when compared against placebo , no treatment, vehicle , or another treatment. We included 60 randomised trials published up to April 2018.
Background
Hand eczema is an inflammation of the skin of the hands that can be caused by contact allergens such as rubber chemicals, but other external factors and atopic predisposition are often important triggers. Hand eczema can cause a reduction in quality of life leading to many work-related problems. Various types of hand eczema exist, and different topical and systemic treatments with unknown effectiveness can be used.
Study characteristics
Most participants were hospital outpatients over 18 years of age with mild to severe chronic hand eczema. Treatment was usually given for up to four months, and outcomes were mainly assessed after treatment. A large variety of treatments were studied and compared to no treatment, variants of the same medication, placebo, or vehicle. Twenty-two studies were funded by pharmaceutical companies.
Key results
Limited data are available to support the best way of managing hand eczema due to varying study quality and inability to pool data from studies with similar interventions. Corticosteroid creams/ointments and phototherapy are the major treatment options, although comparisons between these options are lacking. Below, we present results for the main comparisons of interest.

Hand eczema is often caused or worsened by exposure to a substance thats irritating think alcohol, bleach, cleansers or solvents or one that causes an allergic reaction, such as perfume or certain plants. Other Topical Medication For Eczema
TCIs dont contain steroids. Instead, they control inflammation and reduce eczema flare-ups by suppressing the immune system.
Though TCIs dont cause the same side effects as topical corticosteroids, patients should only use them for short periods of time. A boxed warning alerts patients to the possible cancer risk associated with these drugs.

Another category of topical treatments consists of the calcineurin inhibitors, tacrolimus and pimecrolimus. They also work to reduce inflammation, improve itch, and can combat drynessespecially when used in an ointment formulation.
Its important to talk about these options with your doctor before starting them because they have an FDA black box warning, which was added to the pack- age labeling in 2006 as a response to a strong increase in use of topical calcineurin inhibitors as an alternative to steroids, and there is data suggesting an increased risk of cancer .
I like to have an up-front talk with my patients about this potential risk and explain to them that our use is topical, in limited focal areas, and that I find these topicals to be very safe for long-term use in a controlled manner for eczema. Its important for patients to be informed of this labeling prior to picking up a prescription for the first time and noticing the FDA warning on the packaging.
In many situations, the use of a topical calcineurin inhibitor is preferred over that of a topical steroid. One instance is when the skin has become resistant to steroid use in sensitive areas, such as the eyelid or the lips.
A topical calcineurin inhibitor may also be the best choice when side effects from topical steroids begin to show in the folds of the skin, where you might have too much steroid absorption.
Eczema Treatment: 13 Ways To Find Relief
While there is no cure for eczema, there are a variety of non-invasive eczema treatment options that can provide relief during a flare-up and some that may prevent its onset. These can include corticosteroids, but the following home eczema treatment options may be best.
1. Light Therapy/Phototherapy
According to the National Eczema Association, phototherapy helps to calm inflammation, reduces itching, increases vitamin D production and helps fight bacteria on the skin. Your doctor may prescribe a corticosteroid cream or ointment to apply to your rash. This will help reduce itching and calm inflammation. Use it right after bathing. Follow your doctors directions for using this medicine or check the label for proper use. Call your doctor if your skin does not get better after 3 weeks of using the medicine.
Antihistamines like hydroxyzine reduce itching. They can help make it easier to not scratch. A new class of drugs, called immunomodulators, works well if you have a severe rash.
