What Are The Symptoms Of Scalp Eczema

Symptoms of scalp eczema can vary depending on your age, general health, underlying cause, and severity of the condition. For some, the disease is just a mild nuisance.

For others, it can cause significant symptoms that interfere with daily life.

Depending on the condition causing their symptoms, people with scalp eczema often experience:

  • Greasy patches of thick, yellow, pink, or red scales
  • Flaky skin, or scales that flake off when they are scratched
  • Blisters or bumpy rash

Scalp eczema may not be limited to the scalp.

Symptoms may appear on the head or hairline, on the sides of the nose and cheeks, on the eyelids, eyebrows, ears, upper chest and back, genitals, and underneath the breasts.

An infant with cradle cap may have symptoms on their scalp, face, upper chest, back, and sometimes in the diaper area or armpits.

People who have weakened immune systems are more likely to develop severe cases, with symptoms of eczema appearing on multiple parts of their body.

What Is Seborrhoeic Dermatitis

Seborrhoeic dermatitis is a form of skin inflammation that usually occurs on the scalp, face or torso, in or around areas that naturally produce an oily substance known as sebum.

Seborrhoeic dermatitis is also called seborrhoeic eczema, and can lead to dandruff. If it happens in infants, it causes cradle cap.

What Are The Signs Of Scalp Dermatitis

Scalp inflammation might also be called scalp eczema and typically presents similarly to eczema on other parts of the body, Dr. Debra Wattenberg, who specializes in cosmetic dermatology, told TODAY.

“At the end of the day, the symptoms are almost identical to eczema,” she explained. “You can have itchiness and dryness, and those who are prone to atopic dermatitis can be more itchy on the scalp than those who aren’t.”

“Dermatitis refers to just inflammation of the skin,” said Francis. “Anytime there’s a rash or symptoms like itching or pain or tenderness, that really signifies that there is some kind of inflammation. You can also see redness or flakes. Any kind of flaking of the skin means the skin is shedding off. These symptoms basically define scalp dermatitis.”

Moderate To Severe Psoriasis Treatments

Lost in the Oyster: Scalp Eczema

Your doctor may prescribe steroids, lotions, solutions, sprays, or foams to treat moderate to severe scalp psoriasis. Some topical treatments are applied directly to the skin, then shampooed and rinsed out, including:

  • Anthralin . Apply this cream once a day for 10 to 30 minutes.
  • Calcipotriene . This is a prescription form of vitamin D. Apply it at night and cover your scalp with a shower cap. Leave it on overnight. Dont get it in your eyes.
  • Calcipotriene and betamethasone dipropionate . This is a combination of a type of vitamin D and a strong steroid in a suspension or ointment. You use it once a day. Dont get it in your eyes.
  • Tazarotene . This vitamin A treatment comes in a cream, foam, or gel form. If using it at night, put it on clean, dry skin, and let the medication dry before you go to sleep. Applying a moisturizer after the medication may help prevent drying.

Follow your doctors directions for all medications. Dont use stronger steroids for more than a 2-week cycle without your doctor giving you the OK.

How Is Seborrheic Dermatitis Treated

In teenagers and adults, seborrheic dermatitis usually doesnt disappear on its own without treatment. Type of treatment depends on the area of the body that is affected and how severe your condition is.

The goal of treatment is to reduce the visible signs of seborrheic dermatitis and itching and redness. Treatment includes use of over-the-counter products and prescription products. Prescription products include topical antifungals, calcineurin inhibitors and corticosteroids. Ongoing maintenance treatment is often needed. You may need to use a combination of these treatments:

Scalp Treatment

Cradle cap

  • Cradle cap usually clears up without treatment when the child is between eight and 12 months old. It may be treated daily with a mild baby shampoo.
  • Massage or brush the scalp with a soft brush several times a day and after each shampoo. Be careful not to cause a break in the skin, which can lead to infection.
  • If the problem continues, or the baby seems uncomfortable and scratches the scalp, contact your pediatrician or dermatologist. He or she may prescribe a prescription shampoo or lotion.
  • Other areas of affected skin may be treated with a gentle steroid lotion.

Adolescents and adults:

For moderate to severe cases, your healthcare provider may prescribe a shampoo containing betamethasone valerate , clobetasol , fluocinolone or fluocinolone solution . Shampoo your hair as directed. Some products are used daily, others are used twice daily for two weeks then two times a week.

Shampoos For Head Lice

Itching is one of the most common symptoms of head lice, tiny insects that live on or near a human scalp. The only way to know for sure that a person has head lice is to see one of the lice crawling in the person’s hair. Over-the-counter lice medicine is called pediculicide. Follow the directions on the box carefully.

Tips for head lice treatment:

  • Treat everyone in the household who is infected at the same time.
  • Do not use conditioner on your hair right before applying the lice medication.
  • Do not wash your hair again for 24-48 hours after you rinse out the lice treatment.
  • Wash clothing, pillowcases, sheets, and towels each infected person used in the two days before the lice treatment. Use the hot water cycle on the washer and high heat on the dryer.
  • Soak combs and hairbrushes in very hot water for at least five minutes.

When To Call Your Doctor About Eczema

No matter what type of eczema you might have, schedule a visit if:

  • You get an itchy rash and have a family history of eczema or asthma.
  • The inflammation doesn’t respond within a week to treatment with over-the-counter hydrocortisone creams. You may need stronger forms of treatment.
  • You get yellowish to light brown crust or pus-filled blisters over patches of eczema, especially if youâve been scratching. You could have a bacterial infection that should be treated with an antibiotic. In fact, youâre also more likely to get viral and fungal skin infections.
  • During a flare-up of eczema, you have contact with anyone with a viral skin disease such as cold sores or genital herpes. Having eczema puts you at higher risk of catching the herpes simplex virus.
  • You get many painful, small, fluid-filled blisters in the areas of eczema. You may have eczema herpeticum, a rare but potentially serious complication caused by the herpes simplex virus.

What’s The Difference Between Seborrheic Dermatitis And Psoriasis

Psoriasis and seborrheic dermatitis can mimic each other. Both are patches of red skin with flakes. Both can be found on your scalp and back.

Psoriasis is an inflammatory skin disease that affects 2% to 4% of the population. The scales of psoriasis are often thicker than that of seborrheic dermatitis. The edges of those scales are very well-defined. On the scalp, psoriasis scales are closer to a silver color than white or yellow.

There is a condition called sebopsoriasis where seborrheic dermatitis and psoriasis overlap. In that case you have the symptoms of both: both white flakes and silver flakes, both on the scalp or back, both itchy.

If youre concerned about whether you have psoriasis or seborrheic dermatitis, consult your healthcare provider. Treatment for one may not work as treatment for the other.

What Can Be Done To Treat Scalp Dermatitis At Home

For many, home remedies might be the first step towards treating scalp dermatitis. According to Dr. Hooman Khorasani, the chief of the division of dermatologic and cosmetic surgery at the Mount Sinai School of Medicine, the most common over-the-counter solution is a shampoo with selenium sulfide, a medication that can treat dandruff and some scalp infections. The medication can be found in shampoos like Head & Shoulders, Selsun Blue and others.

“The important thing that people need to know is that you have to leave the shampoo on for usually five or six minutes so it actually works,” Khorasani said. “Especially when you have dermatitis, there’s a thick layer of dead skin, and it’s hard for the medications to kind of penetrate that layer… Anything with selenium sulfide has the ability to take care of it, but the key is to leave it for at least five minutes before rinsing it off.”

Natural remedies like tea tree oil and apple cider vinegar can be used to treat scalp dermatitis, Wattenberg said. Francis added there are also herbal treatments available for those looking to avoid traditional medication. Depending on the type of scalp dermatitis, these topical remedies may not eliminate the root cause.

How Is Cradle Cap Treated

Cradle cap and seborrheic dermatitis in infants usually clears up on its own in weeks or months. In the meantime, you may want to loosen and remove the scales on your babyâs scalp:

  • Wash your babyâs hair once a day with mild, tear-free baby shampoo.
  • Gently remove scales with a soft brush or toothbrush.
  • If the scales donât loosen easily, apply a small amount of mineral oil or petroleum jelly to your babyâs scalp. Let the oil to soak into the scales for a few minutes to several hours, if needed. Then use a soft brush or toothbrush to remove scales. Shampoo your babyâs hair as usual.

If regular shampooing doesnât help, your doctor may recommend a mild steroid cream or antifungal shampoo.

For seborrhea on other parts of the body, your doctor may recommend a mild steroid or antifungal cream.

Do not use over-the-counter steroid or antifungal creams or anti-seborrhea shampoos without checking first with the doctor.

Scalp Eczema Vs Psoriasis

Though both scalp eczema and psoriasis come with itchy and red patches on the scalp, they have slight differences in appearance.

Scalp eczema appears as dry, flaky, and red patches. These patches may sometimes have small bumps which ooze fluid. Also, the affected area becomes sensitive and inflamed due to scratching.

On the other hand, scalp psoriasis is an autoimmune condition in which thick and scaly patches in greyish color are formed on the scalp. These thickened patches are known as ‘plaques’, which may be triggered by microbial infections or stress.

However, both scalp psoriasis and eczema can be triggered by genetic factors. So, if one of these conditions runs in your family, then you are more likely to have it.
