Dermatologist Solves The Case Of The Printing Press Operators Hand Rash
When he was 42 years old, the skin on Marks hand became extremely painful and slightly itchy. A rash covered the back of his hands and some of his forearms. The only time Mark felt some relief was when he was away from work for a week.
Because Mark was exposed to many chemicals, his dermatologist started by testing him for allergies. The results showed that Mark wasnt allergic to anything tested.

A solvent caused this rash on this man’s hands and forearms.
So his dermatologist asked Mark about all the tasks he performed at work. One task stood out. Mark was responsible for cleaning the press between print jobs. The next step was to ask Mark how he used the gloves and how often he changed his gloves. From Marks answers, it seemed likely that the gloves were part of the problem.
During an appointment, Marks dermatologist showed him how to put on and remove the gloves to prevent solvent from getting inside. His dermatologist also recommended that Mark get a new pair of gloves every day rather than once every 4 to 8 weeks.
Related Conditions And Causes Of Dyshidrotic Eczema
As mentioned, atopic dermatitis or contact dermatitis, especially when related to a nickel allergy, can increase your risk of having dyshidrotic eczema, according to the AAD.
Differentiating between the conditions can be a challenge, and sometimes they are related anyway, says Kindred. Atopic dermatitis prefers skin folds, such as the insides of the elbows and the knees. Contact dermatitis is usually within the location that the skin was in contact with the trigger. The confusing part is that both atopic dermatitis and contact dermatitis may be associated with dyshidrotic eczema, confusing the picture.

Treatment For Pompholyx From A Gp
The main treatments your GP may recommend to treat the symptoms of pompholyx are similar to those used when treating atopic eczema, including:
- emollients use these all the time and instead of soap to stop your skin becoming dry
- steroid cream this reduces the inflammation and irritation and helps the skin heal
Your GP will probably prescribe a strong steroid cream to use for a short period of time to minimise the risk of steroid side effects.
You may be advised to wear cotton gloves at night to help the cream sink into the skin.
You can also try:
- soaking your hands in a dilute solution of potassium permanganate for 10 to 15 minutes once or twice a day for up to 5 days

Your lymph nodes are part of your bodys infection-fighting system. They are located throughout the body, including your neck, groin, and armpits. Unless these bean-shaped glands are swollen, you may not be able to notice them.
Swollen lymph nodes, on the other hand, are a sign that your body is fighting an infection. In such cases, you may be able to see and feel the lymph nodes, and they can be tender to the touch. Atopic dermatitis wont cause swollen lymph nodes unless you have an infection as a result of eczema.

Hand eczema flare-ups are beyond annoying. It can cause redness, itching, cracks, blisters, and dryness to the point of peeling and flaking, but thats not all.

Case presentation
A 45-year-old man presents to the emergency department with a two-day history of progressive erythema and swelling affecting both hands . He complains that they are painful and itchy.
The patient has a long history of a chronic scaly rash affecting the palms of both hands but has not had a similar acute rash before. He has been using an antiseptic hand wash for the past seven days because of increasing scale on his palms.
On examination, an erythematous vesiculobullous eruption is observed over the palmar and dorsal surfaces of both the patients hands. The rash extends to the dorsal forearms. Apart from lichenified, scaly skin over the extensor surfaces of his elbows, his skin is normal.
Differential diagnoses
Conditions to consider among the differential diagnoses include the following.

Identification of allergic contact dermatitis is mainly based on clinical suspicion, supported by resolution on withdrawal of the suspected allergen. Swabs of the rash should be performed to identify any secondary bacterial infection. Patch testing with the suspected allergen and the Australian Baseline Series is required to confirm the diagnosis.1

Pompholyx usually starts as intense itching and burning of the skin on the hands and fingers. The palms and sides of the fingers then erupt into tiny itchy blisters that may weep fluid.

The skin can sometimes become infected. Signs of an infection can include the blisters becoming very painful and oozing pus or becoming covered in a golden crust.

What Causes Hand Eczema
Usually the cause is either an externally triggered contact rash or an internally generated skin reaction. Often there is repeated contact with a particular skin irritant that is causing one of these two things to occur. Common skin irritants include:
- Soaps and detergents, especially any with fragrances, as well as liquid soaps
- Water, particularly very hot water with prolonged skin contact and repeated hand washing
- All cleaning supplies
