Why Is It Important To Diagnose Early
The EH eruption may only last two to six weeks but the blisters can leave scars. If the EH is near the eye and left untreated or not treated soon enough EH can lead to cornea infection which can lead to blindness. In very rare cases, other complications can occur such as organ failure and death if the virus spreads to the brain, lungs and liver.
What Are The Clinical Features Of Eczema Herpeticum
Eczema herpeticum starts with clusters of itchy and painful blisters. It may affect any site but is most often seen on face and neck. Blisters can occur in normal skin or sites actively or previously affected by atopic dermatitis or another skin disease. New patches form and spread over 710 days and may rarely be widely disseminated throughout the body.
The patient is unwell, with fever and swollen local lymph nodes.
- The blisters are monomorphic, that is, they all appear similar to each other.
- They may be filled with clear yellow fluid or thick purulent material.
- They are often blood-stained i.e., red, purple or black.
- New blisters have central dimples .
- They may weep or bleed.
- Older blisters crust over and form sores
- Lesions heal over 26 weeks.
- In severe cases where the skin has been destroyed by infection, small white scars may persist long term.
Secondary bacterial infection with staphylococci or streptococci may lead to impetigo and cellulitis.
Severe eczema herpeticum may affect multiple organs, including the eyes, brain, lung, and liver. It can rarely be fatal.

Home remedies for eczema can be simple or complex. The easiest, most effective treatment is to make changes to avoid or remove whatever is causing the allergic reaction. But try not to expect a quick response. Eczema is easier to control than cure.
Here are some things you can try on your own to ease the irritation of eczema.
Change your laundry detergent or fabric softener. Liquid detergents may be less irritating than powders or tablets. Use an extra rinse cycle when you wash to remove residue.
Put on a cool compress. Holding a clean, damp cloth against skin can ease itching.
Take lukewarm showers or baths for no more than 10 or 15 minutes to prevent dry skin. Dry yourself very carefully and apply moisturizing lotion all over your body.
Add colloidal oatmeal to the bath or as a paste on your skin. This finely ground oatmeal helps with itchy, dry skin. Or try a baking soda bath or paste.
A mild solution of bleach and water may ease inflammation and itching, as well as killing the bacteria that can cause skin infections when you have eczema. Add a half-cup of household bleach to a full tub of water, soak for 10 minutes, and rinse. Talk to your doctor before giving this a try because chlorine can cause problems for some people.

Atopic eczema is a common, chronic, itchy skin condition, which usually develops in early childhood.1 It affects around 20% of children and carries an economic burden comparable with that of asthma.2 The original NICE clinical guideline on Management of atopic eczema in children from birth up to the age of 12 years was published in 2007,3 and a review in 2011 confirmed that the recommendations were current at that time.4

Laundry detergent can contain harsh chemicals that aggravate eczema.
Many body washes and cleansers contain detergents, which help provide a soapy lather. Detergents and other lathering agents can dry out the skin, especially in people with eczema.
Bar soaps can also be harsh on the skin because of their alkalinity.
Try using a gentle, no-lather, fragrance-free cleanser. Avoid products with rough particles for scrubbing or exfoliating, as these can further irritate the skin.
Many people with eczema also find that switching to a more gentle, fragrance- or color-free laundry detergent can help improve symptoms.
Additionally, try skipping fabric softener, which lingers on clothes and often contains fragrances and chemicals that can irritate the skin.
Sitting next to a fireplace or near a furnace may feel good, but it can worsen eczema symptoms. The hot, dry air can dehydrate the skin and aggravate the itchiness of eczema.

How Is Eczema Herpeticum Treated
Eczema herpeticum is considered an emergency. This is because it can spread quickly. So it must be treated correctly and quickly. If you or your child are diagnosed with eczema herpeticum you will normally be sent to hospital that day for advice from a specialist. In children it is quite common to be admitted to hospital while having the first few days of treatment. In adults, particularly if the infection is mild, it may be sufficient to take tablets and be reviewed by your doctor.
It is important not to be tempted to put a steroid cream on the eczema herpeticum. This will probably make it a lot worse. Many people have steroid creams at home, or have used them for their eczema in the past: you shouldnt use them for a painful skin condition unless advised by your doctor.
Antiviral medicine such as aciclovir is usually very successful for treating eczema herpeticum. It may be given as a liquid or a tablet. For people who are too unwell to take it as a liquid or a tablet, it can be given by a drip .
Sometimes an antibiotic may be needed as well. This is because the damage to the skin makes you more likely to become infected by the bacteria normally harmlessly present on the skin as well. This is called a secondary bacterial infection.

Associated Findings And Differential Diagnosis
There are many associated findings with AD7â12 . Children with AD also have a tendency to develop secondary viral and bacterial skin infections. Herpes simplex virus infection is known as eczema herpeticum or Kaposi’s varicelliform eruptions13 . The child is usually febrile and develops small, grouped vesicular lesions in the area of eczema, which subsequently spreads to normal skin. These vesicles form small erosions that crust over 24 to 48 hours. Tzanck smears and viral cultures will confirm the diagnosis.
Ninety-three percent of patients with AD have significant Staphylococcus aureus colonization of the skin.14 On occasion impetigo can occur in areas of eczema . Widespread honey-colored crusting, erosions, oozing, follicular pustules, and furuncles suggest staphylococcal infection. Chronic infection may lead to osteomyelitis. Infection with the nephritogenic M strain of streptococcus may lead to the development of glomerulonephritis.
The differential diagnosis of AD includes other eczematous disorders .

Provision Of Emollientsstatement 4
Emollients are the foundation of effective management in eczema, and children should have sufficient quantities for everyday moisturising, washing, and bathing.1 The emollients should always be used, even when the atopic eczema is clear. The prescribed emollient should be unperfumed, and suited to the childs needs and preferences.1,3 If an emollient causes irritation or is not acceptable to a child or their family, then an alternative product should be prescribed.
Co-prescribing different emollients for use at different times of the day or year may be appropriate. Aqueous cream is no longer recommended by many dermatologists as a leave-on emollient, because of concerns about frequency of skin irritation, and following recent MHRA guidance on its use.9
