Eczema herpeticum starts with clusters of itchy and painful blisters. It may affect any site but is most often seen on face and neck. Blisters can occur in normal skin or sites actively or previously affected by atopic dermatitis or another skin disease. New patches form and spread over 710 days and may rarely be widely disseminated throughout the body.

The patient is unwell, with fever and swollen local lymph nodes.

  • The blisters are monomorphic, that is, they all appear similar to each other.
  • They may be filled with clear yellow fluid or thick purulent material.

    How Is Eczema Herpeticum Treated

    Eczema herpeticum is considered an emergency. This is because it can spread quickly. So it must be treated correctly and quickly. If you or your child are diagnosed with eczema herpeticum you will normally be sent to hospital that day for advice from a specialist. In children it is quite common to be admitted to hospital while having the first few days of treatment. In adults, particularly if the infection is mild, it may be sufficient to take tablets and be reviewed by your doctor.

    It is important not to be tempted to put a steroid cream on the eczema herpeticum. This will probably make it a lot worse. Many people have steroid creams at home, or have used them for their eczema in the past: you shouldnt use them for a painful skin condition unless advised by your doctor.

    Antiviral medicine such as aciclovir is usually very successful for treating eczema herpeticum. It may be given as a liquid or a tablet. For people who are too unwell to take it as a liquid or a tablet, it can be given by a drip .

    Sometimes an antibiotic may be needed as well. This is because the damage to the skin makes you more likely to become infected by the bacteria normally harmlessly present on the skin as well. This is called a secondary bacterial infection.

    Associated Findings And Differential Diagnosis

    There are many associated findings with AD7â12 . Children with AD also have a tendency to develop secondary viral and bacterial skin infections. Herpes simplex virus infection is known as eczema herpeticum or Kaposi’s varicelliform eruptions13 . The child is usually febrile and develops small, grouped vesicular lesions in the area of eczema, which subsequently spreads to normal skin. These vesicles form small erosions that crust over 24 to 48 hours. Tzanck smears and viral cultures will confirm the diagnosis.

    Ninety-three percent of patients with AD have significant Staphylococcus aureus colonization of the skin.14 On occasion impetigo can occur in areas of eczema . Widespread honey-colored crusting, erosions, oozing, follicular pustules, and furuncles suggest staphylococcal infection. Chronic infection may lead to osteomyelitis. Infection with the nephritogenic M strain of streptococcus may lead to the development of glomerulonephritis.

    The differential diagnosis of AD includes other eczematous disorders .

The prescribed emollient should be unperfumed, and suited to the childs needs and preferences.1,3 If an emollient causes irritation or is not acceptable to a child or their family, then an alternative product should be prescribed.

    Co-prescribing different emollients for use at different times of the day or year may be appropriate. Aqueous cream is no longer recommended by many dermatologists as a leave-on emollient, because of concerns about frequency of skin irritation, and following recent MHRA guidance on its use.9
