The results of these trials were so incredible that by week 16, most patients with moderate to severe atopic dermatitis either had a 90 percent disease clearance, or even 100 percent disease clearance, said Emma Guttman-Yassky, MD, PhD, Waldman Professor and System Chair of the Kimberly and Eric J. Waldman Department of Dermatology, Icahn School of Medicine at Mount Sinai, and first author of the paper.

    We achieved extremely high clearance rates that are bringing us closer to the amazing clearance rates that we see in psoriasis, said Dr. Guttman-Yassky. Also, the itch improvements already started to be significant within days from the beginning of the trials, and the maximum clinical efficacy was obtained early, at week 4, and maintained to week 16. She also noted that upadacitinib was well tolerated by patients in the doses of the drug15 mg and 30mgand no important safety risks were observed.

    There are ongoing studies for a number of medicines.

    We have patients at Boston Childrens Hospital with both immune deficiencies and eczema, who really benefit from treatment with intravenous immunoglobulin, also called IVIG. When these patients own immunoglobulins are not at sufficient levels, we find that when given infusions of IVIG, their eczema and skin seems to improve. Another drug, Interferon gamma, has been shown to be effective in many trials.

    Unfortunately, it has side effects which sometimes limit its use. And, of course, I want to mention Dupilumab, which is the new medication I recently read about and discussed previously. .

    Currently, on clinicaltrials.gov there are many open studies for atopic dermatitis. Many supplements are being tested for benefit for eczema, along with some new moisturizers.

    Though the pathways are different, we are starting to see whether biologics relevant to psoriasis might be helpful in eczema. There are many more drugs in development.

Steroids are really reliable. They are very effective and most people are going to get better using them. Lio, who runs an eczema clinic in Chicago, says the problem with steroid creams is that they have too many side effects for consistent, long-term use.

    While many children will outgrow eczema or have less severe symptoms over time, some teens and adults continue to struggle with eczemas itch, rash and misery. Its this group that the JAK inhibitors will be used for, the dermatologist explains.

    Ruxolitinib, the JAK inhibitor skin cream, can be used in patients who need something other than a topical steroid, but whose eczema isnt bad enough to require an oral medication. The approval for ruxolitinib, which blocks JAK 1 and JAK 2, is for short-term use in those with mild to moderate eczema ages 12 and up, who are not getting relief from other topical medications.

    The trial data for ruxolitinib suggest it is fairly powerful and could potentially be the next step after steroids, says Lio. Or, in certain patients or certain areas of the body, it could even supplant primary use of topical steroids.

    I usually give it for sensitive areas like the face or the folds, or in people who have a few residual areas that arent responding to another treatment, she says.
