The human body uses chemical messengers called interleukins . ILs allow the different parts of our immune system to communicate and help fight off harmful viruses and bacteria. If you think of a chemical messenger such as IL-4 as a key, and a cell receptor as a lock, a biologic drug works in a similar way to fixing a coin over the keyhole so that the key is unable to get into the lock .

How Well Does Opzelura Work
A clinical trial used as the basis for FDA approval found that 53.8 percent of the teens and adults with atopic dermatitis who used Opzelura had clear skin after eight weeks, compared with 15.1 percent of those who used a nonmedicated placebo cream, according to results announced by Incyte, the company that makes the drug.
A second trial found that 51.3 percent of participants had nearly clear skin after eight weeks of treatment with Opzelura, compared with 7.6 percent of those on placebo.
In addition, these clinical trials found that itching was reduced in more than half of patients using Opzelura, compared with about 15 to 16 percent of those on placebo.
Common side effects that occurred in more than 1 percent of trial participants included diarrhea, bronchitis, ear infections, tonsillitis, and common colds.

The Food and Drug Administration on Tuesday approved a drug to treat people with a serious form of eczema, a potential breakthrough for people who have suffered for years without relief. But it will not come cheap.
The drug, to be called Dupixent, will carry a list price of $37,000 a year, a hefty price tag for patients who are increasingly being asked to pay a larger share of the drugs they take. Still, its price is a bit lower than many other commonly used biologic drugs, such as Humira and Enbrel, that treat other skin diseases.
In an effort to head off another public battle over the soaring cost of some prescriptions, the drug makers Regeneron Pharmaceuticals and Sanofi took the unusual step of directly negotiating with insurers over the price and other details ahead of time. What consumers pay for drugs has come under heightened scrutiny in the last few years, most recently by President Trump, who has promised several times to take drug companies to task over their costly products.
Things have really deteriorated in our industry, said Dr. Leonard S. Schleifer, the chief executive of Regeneron. Theres so much concern over drug pricing that theres an enormous amount of finger-pointing that is going on thats counterproductive. It isnt necessarily incentivizing the right behavior.
The best-selling products Humira and Enbrel, which treat the skin disease psoriasis and other conditions, carry annual list prices of about $50,000.
Is Dupilumab Available On The Nhs
Dupilumab has been approved by the National Institute for Health and Care Excellence for routine use by the NHS in England and Wales, and by the Scottish Medicines Consortium for NHS use in Scotland. The healthcare system in Northern Ireland usually implements NICE guidance. Initially approved for treating eligible adults with moderate to severe atopic eczema, dupilumab is now also available on NHS prescription to treat children with more severe atopic eczema aged 6 years and above.

The Government will invest over $270 million in this new PBS medicine listing with more than 3,600 people expected to benefit each year.
Without PBS subsidy people might otherwise pay around $22,800 a year to access this life changing medicine.

Rapid Itch Improvements Achieved In Patients Taking Systemic Oral Medication In Phase 3 Trials

An oral medication called upadacitinib yielded rapid and significant improvements in patients with moderate to severe atopic dermatitis , also known as eczema, in phase 3 clinical trials, Mount Sinai researchers reported today in The Lancet online.
Patients who received upadacitinib, a Janus kinase inhibitor 1 , showed impressive reductions in all clinical disease measures, as well as in all itch-related outcomes. The results of these trials were so incredible that by week 16, most patients with moderate to severe atopic dermatitis either had a 90 percent disease clearance, or even 100 percent disease clearance, said Emma Guttman-Yassky, MD, PhD, Waldman Professor and System Chair of the Kimberly and Eric J. Waldman Department of Dermatology, Icahn School of Medicine at Mount Sinai, and first author of the paper.
We achieved extremely high clearance rates that are bringing us closer to the amazing clearance rates that we see in psoriasis, said Dr. Guttman-Yassky. Also, the itch improvements already started to be significant within days from the beginning of the trials, and the maximum clinical efficacy was obtained early, at week 4, and maintained to week 16. She also noted that upadacitinib was well tolerated by patients in the doses of the drug15 mg and 30mgand no important safety risks were observed.

How Is Dupilumab Administered
Dupilumab is given by injection under the skin once every two weeks in patients 12 years and above using a pre-filled syringe or pen, and once every four weeks in children aged 6-11 years using a pre-filled syringe.
A single syringe and needle, or pre-filled pen, delivers one dose. Unlike other drug treatments for eczema, patients or caregivers are able to administer dupilumab themselves after having received appropriate training from a healthcare professional. Patients or caregivers are advised to inject into the thigh or abdomen . If a healthcare professional or caregiver is administering the injection, rather than the patient themselves, the upper arm can be used as an alternative injection site. The injection site needs to be rotated and patients must avoid injecting into skin that is tender, damaged or scarred. Dupilumab must be stored in the fridge . If necessary, the pre-filled syringes may be kept at room temperature for a maximum of 14 days. They cannot be refrigerated again once they have reached room temperature.
