Diagnosis Of Eczema And Depression
Eczema is diagnosed by a dermatologist, a healthcare provider specializing in conditions of the skin, hair, and nails. There are no tests to diagnose eczema. Instead, dermatologists use diagnostic guidelines in which a person must have at least three symptoms from two categories, both major and minor factors.
Depression is diagnosed by a mental health provider, such as a licensed therapist or psychiatrist, based on guidelines from the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition .
To be diagnosed with major depressive disorder, a person is required to have at least five of the below symptoms, including at least one of the first two listed, over two weeks:
- Loss of interest or pleasure
- Weight or appetite change
- Fatigue and loss of energy
- Feelings of worthlessness and guilt
- Recurring thoughts of death or suicidal ideation

- Between the ages of 20 and 40.
- Assigned female at birth.
- With a personal or family history of eczema.
- With a personal or family history of contact dermatitis.
- Who develop allergic reactions when they touch an antigen, which is a foreign substance that causes an allergic reaction.
- Who receive immunoglobulin infusions. Intravenous immunoglobulin is an injection of antibodies for people with an immune deficiency.

A Fourth New Eczema Drug
In addition to the three JAK inhibitors, the FDA also recently approved tralokinumab , an injectable biologic medication for adults with moderate to severe atopic dermatitis.
Tralokinumab is the second biologic medication approved for treating eczema, the first being dupilumab. The new biologic is available for adults.
There is a need for multiple treatments as there is no one-size-fits-all approach when it comes to atopic dermatitis, says Block. Its a chronic, heterogenous disease that affects everyone differently.

As such, genetic, environmental, and immunologic factors contribute to its flare-up, but its a priority to repair the skin barrier. According to Dr. Libby, humectants help add moisture back to the skin, while emollients add back oils, lubricating and softening the skin. Occlusive moisturizers work to prevent transepidermal water loss .

Potential For Other Treatments
The investigators also believe that further studying the role of IL-33 in skin health could reveal whether or not etokimab might also be helpful in treating a more varied array of immune conditions.
This notion is based on a series of observations that the investigators made over the course of the small trial. They explain that during the study, they first delivered a placebo injection to the participants.
A week after that, they gave them the injection with etokimab. At 4 days following each of these two injections, the researchers performed an experiment: They injected a placebo substance into the skin of the participants left arms, and house dust mite allergens into the skin of their right arms.
The investigators then took samples of cells and fluid from the site of these injections to analyze them.
They found that at 1 day after receiving the treatment with etokimab, the participants experienced less neutrophil activity in the spots the researchers challenged with allergens. Neutrophils, which are a type of immune cell, are involved in inflammation.
This, they argue, suggests that targeting IL-33 could actually help treat different immune conditions that involve heightened neutrophil activity.
Doing experimental research in humans in crucially important if we are to make advances in treatment, and in this study it was initially surprising to us that the dominant effect of etokimab was reducing neutrophil migration into the skin, he adds.

New Drug Therapy For Young Children With Severe Eczema
A biologic therapy for very young children with a moderate to severe form of a common skin condition has been shown to be safe and effective in an international trial which involved University of Manchester clinical scientists working within the Clinical Trials Facility at Royal Manchester Childrens Hospital.
The study of the drug, dupilumab, in inadequately controlled eczema is the first large-scale randomised double-blind trial of a monoclonal antibody for any skin disease, in patients aged six months to six years.
The international clinical study involved University of Manchester and Manchester University NHS Foundation Trust clinical scientists, and was sponsored by the biotech company, Regeneron Pharmaceutical.
Eczema is a chronic inflammatory skin condition with prevalence of 20 per cent or higher in children younger than six years.
It is characterised by an itchy red rash particularly on the face and the bends of the elbows and knees, as well as an increased risk of skin infection.
The study, published in The Lancet showed that the drug greatly improved the severity of the condition, reducing skin itching and pain within two weeks.
It also significantly improved patients sleep, and the quality of life of patients and their parents.
Doctors expect that the international study of 162 patients will ensure this treatment is approved for British children in the near future, following its adoption in the United States in June this year.

Jaks And Eczema Relief
The first to win FDA approval, ruxolitinib , is a topical cream for mild to moderate eczema. The approval was based on the results of two Phase 3 trials in more than 1,200 teens and adults. After eight weeks of using the cream twice daily, from 51 to 54 percent in the treatment group had clear or almost clear skin, compared to fewer than 15 percent in the non-medicated group.
The two daily pills abrocitinib and upadacitinib are for patients with moderate to severe eczema. Because of the potential for side effects, the FDA says the medications should be reserved for those who have tried other oral or injectable eczema medications, but are not getting enough relief.
In two large Phase 3 trials for upadacitinib, as many as 80 percent of patients with moderate to severe eczema saw their symptoms improve by at least 75 percent after four months.
Dr. Emma Guttman-Yassky, a professor of dermatology at Mount Sinais Icahn School of Medicine in New York City, says the oral JAK inhibitors work quickly, with some of her patients on upadacitinib finding itch relief starting within hours. By four weeks, a majority saw significant improvement in itch and there were also strong results in skin clearing.
We have patients with wounds all over the body who are completely clearing, says Guttman-Yassky, the primary investigator on the upadacitinib trials. This is super meaningful to the patients.
Treating Eczema A Stepped Approach
The recommended first-line treatments for most cases of eczema are emollients and topical steroids. Paste bandages and wet wraps may be a helpful addition for some people, particularly where scratching is a major problem. Sedating-type antihistamines may be useful in helping with sleeping at night . Long-term use is not recommended.
When there is an inadequate response to appropriate strengths of topical steroid, or if these are not tolerated, especially on areas of delicate skin, topical calcineurin inhibitors the cream pimecrolimus or the ointment tacrolimus may be useful.
Treatments for more severe eczema, or additional treatments, include phototherapy, oral steroids, immunosuppressant drugs, a biologic drug and a Janus kinase inhibitor.
Before progressing to additional treatments, it is essential to check that there is no other explanation for the eczema being uncontrolled. The following are examples of questions that should be considered by your doctor, but it is not an exhaustive list:
