Preclinical Evidence From Animal Studies
The role of vitamin E in relieving inflammation and erythema was studied by Hayashi et al. using an NC/Nga mouse model. In their study, mice were subcutaneously injected in their backs with a 4% -TP every other day for 2 weeks. Sensitization was then carried out using 5% picryl chloride solution followed by 0.05% PC to induce AD-like inflammation. Mice treated with PC without vitamin E treatment showed a decrease in vitamin E levels and a marked increase in thiobarbituric acid reactive substance levels at the stage of peak symptom severity. Conversely, changes in thiobarbituric acid reactive substance and vitamin E levels in the skin and serum were significantly inhibited in -TP-treated mice. Furthermore, levels of the antioxidant enzymes superoxide dismutase and glutathione peroxidase increased at peak severity in vitamin E-treated mice, as opposed to control mice. Mice treated with -TP were also found to have decreased levels of inflammation-associated factors such as immunoglobulin E , cyclo-oxygenase-2, tumour necrosis factor- and nuclear factor- .
No adverse effects of vitamin E were observed in the animal studies discussed above. Collectively, vitamin E may aid in ameliorating the symptoms of AD by suppressing OS, inflammation and preventing further deterioration of the skin barrier function by restoring the water-retaining capacity of the SC. A summary of the studies showing the preclinical evidence for vitamin E in animals is presented in Table 2.

Effect Of Ceramide Supplements On The Skin
Ceramide supplements have been studied in small studies in animals26-29 and humans30-33. Though the data is not quite robust yet the results so far are promising in my opinion. These studies have shown that ceramide supplements improve skin barrier function and hydration and reduce fine lines and wrinkles31,32. Ceramide supplements studied in subjects with eczema also showed improvements in their symptoms and reduction of allergy and inflammation in the skin30,33.
However, for now, ceramide supplements are unlikely to be recommended as a first line treatment for dry skin, aging or eczema until there are bigger, randomised controlled trials that would lend greater support to using ceramide supplements.
At present, our understanding and data available on dietary ceramides is also incomplete, which presents another barrier to interpreting these studies on ceramide supplements. We know that ceramides are large molecules that get digested by the small intestine before being transported by the blood vessels34-37. The precise proportions of these oral ceramides in our diet that get eventually assimilated to the skin or instead, get directed elsewhere to other parts of the body are still obscure.
Ceramide nutritional supplements are also likely useful in patients who have dry skin but are unable to use ceramides in their skincare or want the skin of the entire body to be better moisturised .

To investigate the role of vitamin E in the regulation of OS, Masaki et al. cultivated HaCaT cells with -TP for 24 h and measured the level of intracellular glutathione, a crucial protein involved in removing excess intracellular reactive oxygen species. Glutathione levels were reported to increase at every tested -TP concentration, and cells cultured with 50 M -TP exhibited resistance against H2O2. A deeper investigation on the mechanism of action of -TP using DL-buthionine–sulfoximine, an inhibitor of -glutamyl-cysteine synthetase, showed -TP increases intracellular glutathione level via upregulation of the -glutamyl-cysteine synthetase heavy subunit mRNA . In another study, Gehin et al. suggested the preventive effects of vitamin E against deleterious cutaneous cell damage. A water-soluble analogue of vitamin E, Trolox at 100 M, was shown to reduce lipid peroxidation and increase enzyme activities of superoxide dismutase, glutathione reductase and glutathione peroxidase concurrently in glyphosate-based herbicide-treated keratinocytes . These findings strongly suggest the effectiveness of vitamin E in suppressing OS in keratinocytes.
AD is known to be associated with keratinocyte damage and impaired skin barrier function. Transepidermal water loss is the functional biomarker of skin benefit. Parish et al. have shown keratinocytes treated with 20 and 40 M of -TP for 18 days experienced a reduction in TEWL as compared to the controls.
