How To Use The Eye Ointment

Betamethasone eye ointment is generally used 2 or 3 times a day.

However, if you use the eye drops during the day, you’ll only need to use the ointment at night. This is because the effect of the eye ointment lasts longer than the eye drops.

To reduce the risk of betamethasone affecting your sleep, some doctors prescribe eye drops to use during the day and ointment to use at night.

  • Wash and dry your hands.
  • If you wear contact lenses, remove them before you use the eye ointment.
  • Tilt your head back.
  • Gently pull down your lower eyelid.
  • Hold the tube with the nozzle close to your eye and gently squeeze about 1 to 2 centimetres of the ointment into the space between your lower lid and your eye.
  • Close your eye for a few minutes, so the ointment can spread.
  • Repeat for your other eye, if treating both eyes.
  • Wait 15 minutes before putting your contact lenses back in.

Which Topical Steroid Is Usually Prescribed

Creams are usually best to treat moist or weeping areas of skin. Ointments are usually best to treat areas of skin which are dry or thickened. Lotions may be useful to treat hairy areas such as the scalp.

Which strength of topical steroid is prescribed depends upon the condition being treated and how severe the condition is. For example, if you have severe psoriasis, your doctor may prescribe a strong steroid such as betamethasone. But, if you have a mild skin condition on your face, a weak topical steroid is usually prescribed – for example, hydrocortisone 0.5%.

Topical steroid creams are sometimes prescribed as a combined cream together with another ingredient. For example if your inflamed skin has an infection with germs which are bacteria, the steroid cream may also contain an antibacterial medicine. If your inflamed skin has an infection with a yeast , the steroid cream may also contain an antifungal medicine.

    Chronic Or Recurrent Otitis Externa

Some patients present with frequent otitis externa that may be related to water sports, vigorous ear cleaning or chronic dermatitis. A preparation called EarCalm® spray is on sale to the public.

    • Otitis externa can be painful. You can take simple painkillers such as paracetamol or ibuprofen to help relieve the pain.
    • To help stop further bouts of inflammation, try not to let soap or shampoo get into your ear canal when you wash. You can do this when you have a shower by gently placing a piece of cotton wool coated in soft white paraffin into your ear. Do not poke it in.
    • Otitis externa is more common in people who swim regularly compared to non-swimmers. If it is not possible for you to avoid swimming, try to protect your ears as much as possible.

    Infectious Causes Of Itchy Ears

Like the skin in general, the skin of the ear can get an infection.

    • The most common cause is “swimmer’s ear”, where generally speaking, bacteria start growing in an ear canal causing irritation and drainage.
    • Occasionally ears can be infected with fungus . This usually results in a chronically irriated ear that appears infected but does not respond to ordinary antibiotics.
    • The ear can also be infected with ear mites a parasite similar to the dust mites that live in your pillow. Ear mites in particular, are common in cats, and are probably underdiagnosed in humans. Cevik found an ear mite in about 6% of itchy ears. These mites are extremely common in humans– some studies report them in 100% of adults. Blepharitis (inflammation of the eyelids can also be caused by these mites.
    • Viral infections are rare — the most common one is recurrent herpes zoster where small blisters are seen in or on the ear.

    is discussed separately.

    How Is The Diagnosis Made

History taking and physical examination is often all that is required to make a diagnosis of otitis externa. If fever or signs of toxicity are present, perform standard laboratory testing. Gram staining and culture of the discharge may be helpful, particularly when a bacterial or fungal cause is suspected.

    Aural Toilet By Healthcare Professionals

INNOVET® Dermatology Retopix OTO Ear Drops 15ml

A build-up of wax and the flaky skin from patches of eczema can produce a lot of debris in the ear canal and this can easily get infected, especially when water is present. So prevention of ear infection is key.

Manual cleaning of the ear by a healthcare professional is called aural toilet and is often recommended to remove the debris and scale that can build up in the ear canal. It is done under a microscope, either using a pick or hook or using microsuction, an electric suction system.

Microsuction is also recommended if ear wax is truly impacted, and is considered safer and definitely better than flushing the ears with water, especially for people with ear eczema.

Although many GP practices offer ear syringing and/or ear irrigation to remove ear wax, water-based methods are not recommended for people with ear eczema, unless it is very mild. Sometimes regular aural toilet is recommended to prevent wax and eczemadebris build-up.

Aural toilet can be performed by aural care nurses/technicians, or sometimes by specially trained practice nurses. Ask your GP for a referral to an aural care service or to a private audiologist.
