Why Is My Eczema Peeling

Wet Wraps for Eczema

As we mentioned, exfoliative dermatitis could be the bodys reaction to an underlying health condition or trigger. This is what causes the skin cells to turn over, die, or shed too quickly. Skin cells generally regenerate every 27 days.

But the rapid turnover of skin cells, due to conditions like exfoliative dermatitis, can cause the skin to scale or peel.

When To Use Wet Wraps For Your Childs Eczema

In my office I typically start with some very simple supplements and dietary changes to help heal eczema from the inside out. I list several in my article Allergies, asthma and eczema: The Th1/Th2 story

If these simple supplements are not helping, or if the childs eczema is severe to the point of scratching nightly then I will typically recommend the family to start wet wraps as well.

How To Wrap It Right

Nailing your technique will up the chances of the wrap being effective. Wet wraps work best on your extremities, but you could also wrap one around your torso or back if thats where eczema tends to strike. On clean skin, apply your topical prescription medication which is usually a topical steroid . Then wet a piece of fabricthat can be a gauze bandage or even an article of clothing until its damp. Wrap it over the affected area. Make it snug, but not too tight that it affects your circulation. Check with your dermatologist or allergist to make sure the topical steroid prescribed is appropriate for the wet wrap. Sometimes a good moisturizer works as well.

What Are The Benefits Of Wet Wrap Therapy

The goal of the treatment, according to the National Eczema Association, is to rehydrate the skin and make medications soak in more effectively. Brar says it helps restore the skin barrier, reduces water loss, helps heal the abrasions and crusty areas on the skin, and can treat inflammation if the skin is infected. It also serves as a physical barrier, so by covering the area, it can prevent scratching as well, she says.

Wet wrap therapy is often used on children, which parents can administer at home, but it can work for adults, too. Its just a little more cumbersome given the larger bodies and bigger surface area to treat, Brar says.

For severe eczema, wet wraps can be used multiple times each day for several hours at a time. The study by National Jewish utilized the wet wraps two to three times a day. Great improvements were seen over four days time.As the eczema started to clear, the wet wraps were only applied to the affected areas only rather than the whole body.Dr. Boguniewicz of National Jewish warns that proper technique is important and that overuse can do more harm than good. Talk to your medical care team about technique and frequency for your child.

Keep in mind that wet wrapping increases the absorption of whatever ointment, oil or cream you apply to the skin. Be sure that you have tested and benefit from what you chose to apply before using it with a wet wrap! Only use steroids or other prescription cream as directed by your doctor. For more information on wet wraps and the study done by National Jewish Hospital, see their helpful video:

Treatment Of Atopic Dermatitis In Children

The first step in the treatment of atopic dermatitis is to educate families about the importance of sensitive skin care measures. Outlining the pathophysiology of atopic dermatitis often helps patients and parents understand the importance of maintaining the skin barrier. The faulty skin barrier in atopic dermatitis leads to transepidermal water loss and must be replaced. Daily bathing in lukewarm water, the use of appropriate soap, and liberal application of emollient at least twice daily is essential for the treatment of all patients with atopic dermatitis. Bacterial colonization and impetiginization, which can exacerbate dermatitis, also must be addressed. Written materials and individualized instructions should be provided for the patient and family. In our practice, we also provide an educational video outlining wet dressing technique.

Should You Wash Eczema Every Day

The NEA recommends taking at least one bath or shower every day. It also advocates the soak and seal method:

  • Bathe in lukewarm water for 510 minutes using a gentle cleanser. Do not scrub affected skin.
  • Pat skin dry with a towel and leave it slightly damp.
  • Apply prescription topical medication to the affected skin.
  • Within 3 minutes, apply moisturizer liberally all over the body.
  • Wait a few minutes before getting dressed or applying wet wraps to allow the skin to absorb the moisturizer.
  • It is important to avoid soap and soap-based products as they can have a drying effect on the skin.

Is A Bath Or Shower Better For My Eczema

Water is an effective way to put moisture back into the skin, but only if you use lukewarm water, avoid scrubbing and apply a moisturizer within three minutes after bathing or showering. As long as you follow these rules, both bathing and showering are equally effective in keeping the skin barrier healthy and flexible, so that it can better lock in moisture and keep irritants out.

However, too much contact with water or improper bathing can actually cause irritation. Especially if you repeatedly get your skin wet without moisturizing it immediately afterward. This causes your skin to lose its moisture content and become dry and irritated.

People should only use wet wrapping for a few days at a time, otherwise, the skin can break down. Wet wrap therapy can lead to systemic absorption of steroid medications and increase the risk of steroid side effects.

One 2021 study found that the corticosteroid betamethasone 0.1% valerate cream, applied during wet wrap treatment, may lead to adrenal suppression.

Adrenal suppression can occur with topical corticosteroids due to absorption through the skin. During this study, adrenal suppression was generally temporary, often resolving even with ongoing topical steroid use.

How To Do Wet Wraps At Home

Using ‘Wet Wrap’ to Treat Eczema

Wet wraps can be done at home or when traveling, but only after consultation and approval from your health care provider.

You first want to familiarize yourself with the concept and talk to a specialist about it determine if this is the right approach for your child, Boguniewicz cautioned. Overuse can do more harm than good.

Once you get the go-ahead from your doctor, youll want to start with clean, preferably white, cotton clothing or gauze from a roll for the wet layer, and pajamas or a sweat suit on top as a dry layer. If the eczema is on the feet and/or hands, you can use cotton gloves or socks for the wet layer with vinyl gloves or food-grade plastic wrap as the dry layer.

First, moisten the clothing or gauze in warm water until they are slightly damp. Next, wrap the moist dressing around the affected area. Then, gently wrap the dry layer over the wet one. Lastly, carefully put on nighttime clothing so as not to disturb the dressing. Leave wet wraps on for several hours or overnight, being sure not to let them dry out.

Do wet wraps work for treating the itch and pain associated with eczema? Yes! But you should always consult with a health care provider prior to starting wet wrap therapy.

Who Does Wet Wrap Therapy Help

Wet wrap therapy is best for people that have moderate to severe atopic dermatitis.

If your prescription-strength creams or topical medications donât help, your doctor may want you to try wet wrap therapy before stepping up to medication like biologic drugs.

Wet wrap therapy works well during intense flares of atopic dermatitis. When you have severe pain or itching, it hydrates and soothes your skin. Itâs good for small or widespread areas.

You can do wet wrap therapy at home, without a prescription. You put the dressings on your skin after you apply moisturizer and topical medication.

If the atopic dermatitis is on your face, a medical professional trained in wet wrap therapy will use gauze and surgical netting to apply a face wrap.

How Often Should You Wet Wrap

Wet wrapping will help to control a childs eczema, but there are no hard-and-fast rules about how long the bandages should stay on or how frequently they should be applied. Normally a dermatological healthcare professional should be consulted for advice. If the skin is particularly bad, you could wet wrap every day and overnight until it settles down. Then you could wrap about three times a week. As you get familiar with the process, youll discover the correct intervals to keep the eczema under control.
