Indications And Usage For Loprox Cream

LOPROX® Cream is indicated for the topical treatment of the following dermal infections: tinea pedis, tinea cruris, and tinea corporis due to Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, and Microsporum canis candidiasis due to Candida albicans and tinea versicolor due to Malassezia furfur.

How Is This Medicine Best Taken

Use ciclopirox cream as ordered by your doctor. Read all information given to you. Follow all instructions closely.

• Use as you have been told, even if your signs get better.
• Do not take ciclopirox cream by mouth. Use on your skin only. Keep out of your mouth, nose, and eyes .
• Wash your hands before and after use. Do not wash your hands after use if putting this on your hand.
• Clean affected part before use. Make sure to dry well.
• Put a thin layer on the affected skin and rub in gently.
• Do not use coverings unless told to do so by the doctor.

What do I do if I miss a dose?

• Put on a missed dose as soon as you think about it.
• If it is close to the time for your next dose, skip the missed dose and go back to your normal time.
• Do not put on 2 doses or extra doses.

How To Use Ciclopirox Olamine Cream

Use this medication on the skin only. Clean and thoroughly dry the area to be treated. Apply a thin layer of the medication on and around the affected area and gently rub into the skin, usually twice daily or as directed by your doctor. Wash your hands after using unless the area being treated includes the hands. Do not wrap, cover, or bandage the area unless directed to do so by your doctor.

Do not apply the medication in the eyes, nose, or mouth, or inside the . If you do get the medication in those areas, rinse with plenty of water.

Use this medication regularly in order to get the most benefit from it. To help you remember, use it at the same times each day.

Continue to use this medication for the full time prescribed, even if symptoms disappear after a few days. Stopping the medication too early may result in a return of the infection.

Your condition should start to improve within the first week of treatment. Tell your doctor if your condition worsens or does not improve after 2 weeks for pityriasis and after 4 weeks for other fungal skin infections.

Antiviral Therapy In The Oral Cavity

Herpesviruses causes a variety of oral mucosal lesions, including herpetic gingivostomatitis, recurrent intraoral herpes simplex, herpes labialis, herpes zoster , or the life-threatening eczema herpeticum. Herpetic gingivostomatitis can also manifest in weakened hosts as aphthoid of Pospischill-Feyrter, characterized by rapid expansion of the lesion to the throat and perioral skin. The majority of these HSV-associated viral lesions are routinely treated by oral acyclovir, with intravenous administration in some severe cases. Treatment options for herpes labialis are listed in Table 34-10.

Acyclovir is best used as soon as the symptoms begin to appear. The mechanism of action of acyclovir is depicted in Figure 34-8. The intravenous dosage is based on body weight and the type of the lesion. Generally, 5 to 10 mg/kg of body weight is administered intravenously for a 1-hour period and repeated every 8 hours for 5 to 10 days. Long-term suppressive acyclovir therapy is recommended for patients with eczema herpeticum at 200 to 400 mg orally two to three times per day. In addition, supportive therapies for herpetic lesions include antipyretic analgesics, as well as antibiotics and antifungals that help control secondary infections.

Seborrheic dermatitis is a common inflammatory skin disease, affecting between 1% and 3% of immunocompetent adults. While its cause is unknown, a number of predisposing factors have been reported, including the implications of Malassezia yeasts. Various treatment options are available, such as ciclopirox shampoo, which combines anti-Malassezia activity with an anti-inflammatory action. Clinically, seborrheic dermatitis presents as red, flaking, slightly greasy-looking patches, which are located primarily on the scalp, nasolabial folds, ears, eyebrows and chest. The degree of flaking and erythema can vary markedly between patients.1 The exact cause of seborrheic dermatitis is not known, however, a number of predisposing factors have been reported. Such endogenous host factors include nutritional,2 environmental,3,4 and immunological5 factors, as well as the presence of Malassezia yeast.6

What Other Drugs Will Affect Ciclopirox Topical

Tell your doctor about all your current medicines and any you start or stop using, especially:

• an inhaled or topical steroid medicine or

• seizure medication

It is not likely that other drugs you take orally or inject will have an effect on topically applied ciclopirox. But many drugs can interact with each other. Carcinogenesis Mutagenesis Impairment Of Fertility

A carcinogenicity study in female mice dosed cutaneously twice per week for 50 weeks followed by a 6-month drug-free observation period prior to necropsy revealed no evidence of tumors at the application site.

The following in vitro and in vivo genotoxicity tests have been conducted with Ciclopirox olamine: studies to evaluate gene mutation in the Ames Salmonella/Mammalian Microsome Assay and Yeast Saccharomyces Cerevisiae Assay and studies to evaluate chromosome aberrations in vivo in the Mouse Dominant Lethal Assay and in the Mouse Micronucleus Assay at 500 mg/kg .

The following battery of in vitro genotoxicity tests were conducted with Ciclopirox: a chromosome aberration assay in V79 Chinese Hamster Cells, with and without metabolic activation a gene mutation assay in the HGPRT – test with V79 Chinese Hamster Cells and a primary DNA damage assay ). An in vitro Cell Transformation Assay in BALB/C3T3 Cells was negative for cell transformation. In an in vivo Chinese Hamster Bone Marrow Cytogenetic Assay, Ciclopirox was negative for chromosome aberrations at 5,000 mg/kg.
