Approximately one quarter of cases of chroniceczematous cheilitis are due to allergic contact. The rest are due to constitutional and irritant factors.

Who Gets Eczematous Cheilitis And Why
Eczematous cheilitis can be divided into two major groups based on the cause:
- Endogenous, meaning it is due to an inherent characteristic of the person. Atopic dermatitis is the most common example of this form of eczematous cheilitis.
- Exogenous, meaning it is due to something from outside the body. This group can be further subdivided into:
- Irritant contact cheilitis lip-licking is the major cause of this type of eczematous cheilitis. Other common causes of irritant cheilitis include cosmetics, foods and environmental factors.
- Allergic contact cheilitis a relevant allergic reaction is identified in at least one-quarter of cases of eczematous cheilitis. Common sources of allergens include lipsticks and other lip care products, toothpaste and dental materials, foods, medications and nail varnish.
Often a combination of factors is present.
Rarely, contact urticaria can present as cheilitis when the allergen is used in small amount frequently, such as the flavouring in a toothpaste.
Sometimes no cause can be identified.
Atopic Eczema Around The Eyes
The primary difference separating atopic eczema from contact eczema is the cause. Atopic eczema is not due to an allergy but rather to atopy, a genetic hypersensitivity to the environment. The eyelids are a common area for atopic dermatitis to appear. It can sometimes be tough to distinguish between all these conditions, from allergic conjunctivitis caused by airborne allergens to allergic contact eczema.
Atopy can manifest as eczema as well as asthma, rhinitis, conjunctivitis, and more.

How Is Allergic Contact Cheilitis Diagnosed
Patch testing is the key to the diagnosis of contact cheilitis. Testing should include the baseline series of patch test allergens as well as cosmetic and toothpaste series, and others suggested by the history. It is most important to also test the patient’s own products and musical instruments if possibly relevant. Cosmetics are often applied as is, but sawdust from wooden instruments should be applied diluted to 10% in petrolatum. A significant number of patients react only to their own products. The relevance of positive results must be assessed, based on careful history taking and clinical examination. Multiple positive reactions are common.
Positive patch tests
Repeated open application test or start-restart testing may be required for a patient’s own products due to irritation under occlusion in patch testing, such as with toothpastes.
may also be useful when investigating cheilitis when routine patch testing is negative.
Should contact urticaria of the lip be suspected, prick/scratch testing is required.
It is common for patients with allergic contact dermatitis to have a second diagnosis such as atopic cheilitis or irritant contact cheilitis.
Common Symptoms Of Eyelid Dermatitis
With either type of eyelid dermatitis, you may experience symptoms in one or both eyes. Sometimes symptoms occur occasionally other times, eyelid eczema is chronic.
Common symptoms include:
- Red, scaly rash on your eyelids
- Itching and swelling around your eyes
- Pain or burning in and around your eyes
- Irritated skin
It may be hard to know whats causing these symptoms to flare up or become chronic, but its usually the result of an allergen or sensitivity to something in your environment.

What Are The Symptoms Of Eczema On Eyelid
The symptoms of eczema on eyelid depend on the source of allergy and the time of exposure. However, there are some general symptoms encountered in all patients and these are:
- Intense itching, especially at night
- Redness of the affected outer skin of the eyelid
- Red and watery conjunctiva
- Lesions of various forms, sizes and shapes macules, papules, vesicles or blisters
- Chronic and constant exposure to an allergenic substance can lead to the following symptoms:
- Itchiness present but less intense
- The eyelid skin is dry and scaly
- The affected area becomes thickened in time.
