Jaks And Eczema Relief
The first to win FDA approval, ruxolitinib , is a topical cream for mild to moderate eczema. The approval was based on the results of two Phase 3 trials in more than 1,200 teens and adults. After eight weeks of using the cream twice daily, from 51 to 54 percent in the treatment group had clear or almost clear skin, compared to fewer than 15 percent in the non-medicated group.
The two daily pills abrocitinib and upadacitinib are for patients with moderate to severe eczema. Because of the potential for side effects, the FDA says the medications should be reserved for those who have tried other oral or injectable eczema medications, but are not getting enough relief.
In two large Phase 3 trials for upadacitinib, as many as 80 percent of patients with moderate to severe eczema saw their symptoms improve by at least 75 percent after four months.
Dr. Emma Guttman-Yassky, a professor of dermatology at Mount Sinais Icahn School of Medicine in New York City, says the oral JAK inhibitors work quickly, with some of her patients on upadacitinib finding itch relief starting within hours. By four weeks, a majority saw significant improvement in itch and there were also strong results in skin clearing.
We have patients with wounds all over the body who are completely clearing, says Guttman-Yassky, the primary investigator on the upadacitinib trials. This is super meaningful to the patients.

These include pills such as the oral steroid prednisone, oral immunosupressants such as cyclosporin, the injectable biologic drug dupilumab, or phototherapy.
The oral JAK inhibitors efficacy is among the most impressive Ive ever seen, says Lio. Although as one might expect, this power does not come without the potential for side effects.
And while there are similarities between abrocitinib and upadacitinib in that they act on the JAK pathways, these medications are not interchangeable, says Dr. Jonathan Silverberg, an associate professor of dermatology at George Washington University School of Medicine and Health Sciences.
No two JAK inhibitors are exactly alike, says Silverberg, a lead author on one of the main abrocitinib studies. Part of the excitement is to start using them and to gain more insight into where those differentiations are.

More Jak Inhibitors For Eczema Are On The Horizon
Cibinqo , an oral JAK inhibitor from the drugmaker Pfizer, won marketing approval from drug regulators in the United Kingdom in September 2021, according to a press release shared on Business Wire. The FDA extended its review of the drug in July 2021 in light of rheumatoid arthritis safety issues seen with another JAK inhibitor, Xeljanz , the company said.
The oral JAK inhibitor Olumiant is approved for atopic dermatitis in Europe and Japan and for rheumatoid arthritis in the United States, the drugmaker Eli Lilly reported in April 2021.
The oral JAK inhibitor Rinvoq is approved for atopic dermatitis in Europe and rheumatoid arthritis in the United States. In April 2021 the FDA extended its review of Rinvoq for atopic dermatitis to get more information on the benefit-risk profile, drugmaker AbbVie said.
Topical JAK inhibitors provide the opportunity to manage more mild disease without the same potential side effects from oral therapies, Dr. Smith says.

Why Does Opzelura Have A Black Box Warning
Along with its approval of Opzelura, the FDA issued a so-called black box warning reserved for medications with the most dangerous potential side effects, based on the risk of serious infections, cancer, events like heart attack and stroke, and death.
These risks were not seen in clinical trials of Opzelura, but only in trials of oral JAK inhibitors in patients with rheumatoid arthritis, says Saxon Smith, MBChB, PhD, a dermatologist and clinical associate professor at the University of Sydney in Australia.
There have been significant concerns relating to oral Janus kinase inhibitors, leading to the black box warning placed on topical Opzelura, he explains.
Topical JAK inhibitors should have a lower risk of serious side effects because, unlike pills that send medication throughout the body, creams and ointments apply therapy only to a targeted area on the skin, says Thomas Bieber, MD, PhD, a professor and the chair of dermatology and allergy at the University Hospital of Bonn in Germany.
Still, the black box warning may make some doctors and patients reluctant to try Opzelura. There will be some measure of caution at first as this medication is a first of its kind, and more studies may be needed to better assess its efficacy, safety, and tolerability with long-term use, says the University of Pennsylvanias Fuxench.

The ingredient, which is made of ground-up oat kernels, helps relieve the common side effects of eczema such as inflammation and itchiness as well as keep your skin barrier intact to prevent future damage and moisture loss.
