Dupixent For Chronic Rhinosinusitis With Nasal Polyps

Dupixent is approved to treat chronic rhinosinusitis with nasal polyps in adults. Its used when the condition is not well controlled by other treatments.

Dupixent is used as an add-on treatment in people with chronic rhinosinusitis with nasal polyps who are already taking another medication to treat it. Rhinosinusitis is when your sinus cavities swell and cause pain or pressure in your face, along with congestion. Rhinosinusitis becomes chronic when it lasts for 12 weeks or longer.

Nasal polyps are tissue growths in the nose that are noncancerous. People with chronic rhinosinusitis usually have an increased number of eosinophil cells in their nasal and sinus cavities. Eosinophils are immune system cells that increase swelling in the area.

Dupixent helps by reducing swelling in the area, which relieves symptoms of chronic rhinosinusitis with nasal polyps.

In clinical trials, Dupixent greatly decreased nasal and sinus symptoms caused by chronic rhinosinusitis. It also helped improve peoples sense of smell. Reduced ability to smell is a common side effect of nasal polyps.

After 24 weeks, people taking Dupixent experienced a 51% to 59% decrease in nasal congestion. In comparison, people taking a placebo experienced a 16% to 18% decrease in nasal congestion.

What Are The Causes Of Eczema

Best Eczema Creams and Lotions to Soothe Dry, Itchy Skin

The exact causes of atopic eczema are not known, but it does occur more often in people who are prone to allergies, and also those who suffer from other problems such as asthma and hay fever. It has been established that eczema runs in families, so genetics has a part to play in whether or not youll be affected by the condition. If you have genes that predispose you to eczema, the condition may then be triggered by allergens in the environment such as dust, house dust mites or pollen. In many cases, some types of food can act as allergens that trigger eczema milk, eggs, nuts and wheat are common culprits.

However, allergens are not the whole story, as eczema can also be triggered by other factors including cold weather, rough clothing, harsh soaps, washing too much, stress and sweating a lot. In general, it does seem that the prevalence of eczema is on the increase. This may be due to changes in the way we live or the environment we live in, but the relative importance of each factor is not clear.

If Im Taking Dupixent For Asthma Do I Still Need To Use My Rescue Inhaler

Yes, you should still have a rescue inhaler even if youre treating your asthma with Dupixent. Dupixent is used to prevent asthma attacks from happening. However, if you have an asthma attack, you need a rescue inhaler to help open up your lungs quickly so you can breathe. Never use Dupixent to treat an asthma attack.

Talk with your doctor if while taking Dupixent your breathing worsens, your rescue inhaler isnt working, or you need to use your rescue inhaler more often than usual.

How Well Does Opzelura Work

A second trial found that 51.3 percent of participants had nearly clear skin after eight weeks of treatment with Opzelura, compared with 7.6 percent of those on placebo.

What Ingredients To Avoid In Eczema Creams

Of course, there are ingredients youll want to avoid too, Dendy Engelman, M.D., a board-certified dermatologist at Medical Dermatology and Cosmetic Surgery, tells SELF. Ingredients to watch out for include fragrances, exfoliating acids , harsh soaps, and retinol, she says: These can further irritate the skin by drying them out.

Dr. Nada Elbuluk, M.D., director of the Skin of Color Center and Pigmentary Program at USC, also notes to look at ingredients lists for anything you might be allergic to, because that will also cause a flare-up. I also recommend avoiding physical exfoliants and even certain fabrics such as wool, which can make eczema worse, she says.

So what are the absolute best eczema creams to hydrate, strengthen, and protect against flare-ups? We asked dermatologists to tell us what moisturizers they commonly recommend to their eczema patients. Heres what they suggest.

What If I Miss A Dose

What to do if you miss a dose of Dupixent depends on how often you take the drug.

For instance, if you take Dupixent every other week and miss a dose, take the dose within 7 days of missing it. If you forget to take it within 7 days, skip that dose and wait until youre supposed to take the next dose.

For children taking Dupixent once every 4 weeks, give the missed dose within 7 days of when it was missed. Then, go back to their original dosing schedule. If the missed dose isnt given within 7 days, give one dose, then start a new dosing schedule based on the date its given.

For example, lets say your child usually takes a dose of Dupixent on the first of the month. But this dose is missed, and its not taken until the eighth of the month. In that case, your child will start taking a dose on the eighth of every month.

To help make sure that you dont miss a dose, try setting a reminder on your phone. A medication timer may be useful, too.

What Are My Options When It Comes To Changing My Atopic Dermatitis Treatment

Once youve talked to your doctor about any concerns youre having with your atopic dermatitis/eczema treatment, there are a few options you can explore together. Your doctor may, for example:

  • Tweak the dosage of your medication. Many people with eczema often start with topical treatments, and doctors can increase the strengths of those medications if needed, according to Piliang. Sometimes it may be just going to a stronger topical medication for flares, she says.
  • Recommend supplementing your current treatment plan with another drug. In some cases, if your symptoms are no longer well controlled, your doctor may step up therapy to a more aggressive treatment, says Piliang. We may add systemic medications, such as biologics and JAK inhibitors, she adds.
  • Recommend switching to a new treatment altogether. There are times when its appropriate to switch treatments, according to Piliang. This can include situations in which your symptoms arent well controlled or you may be experiencing unpleasant side effects with your current regimen.
  • Ask you to do some trial and error to find the right treatment or combination of treatments. It can take some time to find the right eczema treatment for you so you shouldnt feel bad about telling your doctor if a medication isnt working, says Piliang.

What Other Information Should I Know

Keep all appointments with your doctor and the laboratory. Your doctor may order certain lab tests to check your body’s response to triamcinolone.

Do not let anyone else use your medication. Ask your pharmacist any questions you have about refilling your prescription.

It is important for you to keep a written list of all of the prescription and nonprescription medicines you are taking, as well as any products such as vitamins, minerals, or other dietary supplements. You should bring this list with you each time you visit a doctor or if you are admitted to a hospital. It is also important information to carry with you in case of emergencies.

How To Inject Dupixent

Dupixent is given as a subcutaneous injection . You should take Dupixent according to your doctor or pharmacists instructions.

For your first dose of Dupixent for treating eczema or asthma, you will typically get two injections in two different areas of your body. Usually, the first dose is given at your doctors office. After the first dose, most people only need one dose every other week.

For treating chronic rhinosinusitis with nasal polyps, treatment starts and continues with one dose every other week.

Your doctor can teach you how to give yourself injections, or you can go to their office for injections. For children, its recommended that the doctor gives the injections in their office.

Dupixent syringes and pens should be stored in the refrigerator at 36 to 46°F .

Before using Dupixent, remove it from the refrigerator and let it reach room temperature. Depending on the form of Dupixent youre using, youll need to leave it out of the fridge for either 30 minutes or 45 minutes.

Never heat Dupixent syringes or pens, or put them in the sunlight.

Dupixent can be injected into your thigh, upper arm, or stomach. Dupixent shouldnt be injected within 2 inches of your belly button. Use a different area each time you inject, and dont inject into skin thats bruised, damaged, tender, or scarred.

Applying The Correct Dose

Dermatologists use the fingertip unit to help gauge how much topical corticosteroid should be applied to the affected area of the body. A person’s age and the size of the surface area will determine the FTU dosing.

One FTU is the length of the tip of the adult finger to the first crease and is typically about 0.5 grams.

How Should Eczema Cream Be Used

Mom starts business to treat daughters eczema

Different eczema creams are used in different ways. How and when you apply your eczema cream will depend upon whether youre going through a flare-up, how bad your symptoms are, and where on your body the affected areas are.

If youre not going through a flare-up of symptoms, or if your eczema is very mild, you should simply use emollients. Emollients should be used every day at least twice, but ideally more often, especially if you have very dry skin. You should use a large amount and apply it all over your skin, smoothing it in the same direction your hair grows.

If you are experiencing a flare-up, you should use emollients and topical corticosteroids. Apply a layer of emollient first, wait for it to soak into the skin, and then use your corticosteroid cream or lotion. This should be applied only to the affected areas, as directed in the products patient information leaflet. Typically, you will have to apply corticosteroids once or twice a day.

Other things to consider include the following:

You should always speak to a pharmacist or doctor before you start using eczema creams, to make sure youre using the right kind at the right strength.

What Are The Side Effects Of Azathioprine

It is important to remember that most patients do not experience significant side effects. Blood tests will be taken before treatment to assess the individual risk for developing side effects. The main side effect concern with azathioprine is bone marrow suppression. This can result in severe anaemia and risk of infection. A blood test is now available to determine which patients are most at risk of developing this side effect with azathioprine. About 1 in 200 people have very low levels of an enzyme called thiopurine methyltransferase . They are unable to break down azathioprine in the normal way and are at high risk of dangerous bone marrow suppression. These patients should not receive azathioprine. A further group of patients who have intermediate activity of TPMT may be given azathioprine, but it is recommended that a reduced dose be given, starting at 0.5 mg/kg and increasing to 1.0 mg/kg. If the level of TPMT is normal, risk of developing side effects is low. All patients taking azathioprine require regular blood tests before and during treatment to check for any signs of bone marrow suppression even if their TPMT level is normal. You should seek medical advice if you become unwell or develop signs of infection. It is important to report any unusual bruising or bleeding, which may be a sign that the bone marrow is being affected.

Nausea, diarrhoea and loss of appetite may be a problem for some patients, but this usually responds to a reduction in dose.

Why Does Opzelura Have A Black Box Warning

Along with its approval of Opzelura, the FDA issued a so-called black box warning reserved for medications with the most dangerous potential side effects, based on the risk of serious infections, cancer, events like heart attack and stroke, and death.

These risks were not seen in clinical trials of Opzelura, but only in trials of oral JAK inhibitors in patients with rheumatoid arthritis, says Saxon Smith, MBChB, PhD, a dermatologist and clinical associate professor at the University of Sydney in Australia.

There have been significant concerns relating to oral Janus kinase inhibitors, leading to the black box warning placed on topical Opzelura, he explains.

Topical JAK inhibitors should have a lower risk of serious side effects because, unlike pills that send medication throughout the body, creams and ointments apply therapy only to a targeted area on the skin, says Thomas Bieber, MD, PhD, a professor and the chair of dermatology and allergy at the University Hospital of Bonn in Germany.

Still, the black box warning may make some doctors and patients reluctant to try Opzelura. There will be some measure of caution at first as this medication is a first of its kind, and more studies may be needed to better assess its efficacy, safety, and tolerability with long-term use, says the University of Pennsylvanias Fuxench.
