Eczema affects nearly 32 million people in the United States, and it impacts all skin colors and ethnicities. But if you search online for eczema symptoms, youll likely read descriptions and find photos of bright red, bumpy, inflamed patches on light skin.
In people of color, eczema can be uniquely different in appearance. It is often hard to identify because the redness may be obscured in darker skin. Instead, eczema may present as dark brown, purple or ashen gray. As a result, misdiagnosis or delayed diagnosis can occur.
Studies show eczema and atopic dermatitis are more common in Black Americans, especially in children. They may have more itchiness and inflammation that requires higher doses of medications to get relief. Black and Hispanic children tend to develop more severe cases of atopic dermatitis compared to white children. They are also more likely than white children to miss school due to eczema.

How Common Is Eczema In Skin Of Color
Eczema can occur in anyone, regardless of ethnicity. However, it is more common in certain populations. In the United States, 19.3 percent of Black children have eczema, compared to 16.1 percent of white children and 7.8 percent of Asian children. Black children are 1.7 times more likely to develop eczema than white children.3
Eczema typically starts in childhood, but adult-onset disease does occur.
How Can You Treat Eczema In Black Skin
There are a few ways experts treat eczema, but it depends on the severity of the case. Moisturizers are one of the most important treatments, Dr. Scott says. We need to restore lost water and seal it in, so thick creams or ointments are the backbone to any eczema treatment and preventing flare ups. Consider over-the-counter options like Vanicream Moisturizing Cream or CeraVe Moisturizing Cream.
While a heavy moisturizer can help keep moisture locked into the skin, Dr. Scott says a flare-up will often require a topical steroid. Topical steroids calm down the inflammation and help to control itch, but shouldnt be used long-term. If youre looking for a non-steroid option, talk with your dermatologist to see which option would be best for you. For severe cases that affect most of the body, one treatment option is a light-based treatment called phototherapy. Oral medications or biologic injections might be other possibilities, Dr. Scott adds.
While all of these treatments are the same no matter what race or ethnicity you are, Dr. Kagha and Robinson both noted that a common concern among brown-skinned patients is hyperpigmentation once the flare-up has been treated. Dr. Kagha says that these post-inflammatory spots are often left behind once the eczema fades, but these marks can be treated with a prescription retinoid, for example, and should fade with time.

Keep The Skin Moisturized
When youre treating hyperpigmentation, your main goal is to lighten any dark spots. You can find over-the-counter products with ingredients that will even your skin tone. However, you should also look for creams and potions that contain hyaluronic acid or glycerin to infuse the skin with moisture. A product that lightens the skin but also contains retinol is also ideal for prompting cell turnover and give you a healthy glow. Following your skin treatments with a moisturizer that replenishes the fat barrier of your skin and protects you from UV rays.

Understanding Different Kinds Of Eczema Pigmentation
Inflammation stimulates the activity of melanocytes, the cells that give skin their color, boosting the production of skin pigment and causing darker patches of skin, or hyperpigmentation. Every time you scratch your skin, more melanin is released and the patch of skin gets darker. Since those with darker skin have more melanin, the reaction causes even more pigment production.
Darker patches of skin are a result of post-inflammatory pigmentation or lichenification.
Post-inflammatory pigmentation: After an eczema flare subsides, skin might be darker in the places where eczema lesions have healed. This is called post-inflammatory pigmentation and itâs also more common in darker skin .
While the dark patches caused by post-inflammatory pigmentation will eventually fade, they can remain on the skin for months and may last even longer. Research shows that post-inflammatory pigmentation lasted up to 5 years in more than 22% of diagnosed cases. Daylight can make post-inflammatory pigmentation worse, so sun protection is essential.
Lighter patches of skin are called pityriasis alba.
Eczema pigmentation is often mistaken for scarring. If eczema patches become infected, they can leave scars, but hypopigmentation and hyperpigmentation have a totally different cause than scarring the pigment changes affect only the upper layer of the skin and donât leave scars. Unlike scars, which are permanent, pigment changes are temporary and often resolve on their own.

What Does Eczema Look Like On Black Skin
Since Black people come in an array of shades, the appearance of eczema can vary based on skin tone. However, there are some tell-tale signs that the condition is present.
“Eczema on Black skin presents itself as ashen grey, purple or violaceous, red-brown or dark brown,” Dr. Okereke explains. “On adults, it can present itself on the inner or outer arms, crux of the elbow, neck, eyelids, and hands to name a few. On infants, it may appear on the cheeks, neck, and hands.”
The MD adds that while most commonly, eczema is thought to show up as patches of dryness, it can pop up in other forms as well. “It can appear as little bumps, papules, thickened plaques, lichenoids, or what we call a follicular presentation, and this presentation is more common in persons with darker skin tones,” she says.
Which Symptoms Are Similar Among Skin Tones
- Itching: It may be bad enough to disrupt your focus at school or work.
- Skin thickening from scratching: This may appear very dark on darker skin tones.
- Scratches: These may show up as marks or cuts and could lead to infection.
- Swelling: Fluid buildup or infection usually causes this.
- Oozing: This is often the result of seeping or leaking of fluid from edema.

Symptoms Of Discoid Eczema
Discoid eczema causes distinctive circular or oval patches of eczema. It can affect any part of the body, although it does not usually affect the face or scalp.
The first sign of discoid eczema is usually a group of small spots or bumps on the skin. These then quickly join up to form larger patches that can range from a few millimetres to several centimetres in size.
On lighter skin these patches will be pink or red. On darker skin these patches can be a dark brown or they can be paler than the skin around them.
Initially, these patches are often swollen, blistered and ooze fluid. They also tend to be very itchy, particularly at night.
Over time, the patches may become dry, crusty, cracked and flaky. The centre of the patch also sometimes clears, leaving a ring of discoloured skin that can be mistaken for ringworm.
You may just have 1 patch of discoid eczema, but most people get several patches. The skin between the patches is often dry.
Patches of discoid eczema can sometimes become infected. Signs of an infection can include:
- the patches oozing a lot of fluid
- a yellow crust developing over the patches
- the skin around the patches becoming hot, swollen and tender or painful
- feeling sick

Once we identify nummular eczema and treat it appropriately, most people are significantly better within seven to 10 days, and, for patients who dont have another type of eczema, it often never comes back, according to Dr. Peter Lio, founding director of the Chicago Integrative Eczema Center. Although eczema often starts in infancy and childhood, most research is done in adults with eczema. This means research might be missing the earliest triggers of eczema. There is also little research on how well typical therapies for eczema work on non-white skin.1,2

Does Eczema Look Different On Brown Skin
Like we stated before, not all eczema is the same, but there are certain symptoms that are common. In general, eczema rashes are very itchy, dry patches on the skin, says Dr. Scott. On lighter skin, they often appear red, but on darker skin they can appear purplish or brown.
But not every dry patch that appears on the skin is actually eczema. Dr. Robinson notes that if the patches arent itchy, the diagnosis of eczema is a less likely one. So if you find that itch is consistent in your dry patches, its best to see a dermatologist as soon as possible.
Other symptoms Dr. Robinson notes are scaling, crusting, scratches, thickening of the skin, and redness. But she does note that redness can be a challenge to detect on dark skin, and has the potential to lead to underdiagnosis or misdiagnosis.
Consistency And Ease Of Application
This is subjective. But it would be fair to say that most parentsprefer sunscreens that arent too thick and greasy and are non-staining.
The caveat is that every physical sunscreen that containsnon-nanosized particles of zinc oxide or titanium dioxide will leave anoticeable white coating on the body. Its the old school variety of sunscreen.Simple but effective.
The extent of the visibility of this layer varies from one brand tothe other. Some sunscreens disappear if you rub it gently into the skin. Theseare better suited for daily use, considering that children can go outsidewithout a white chalky layer on their face.
