What It Feels Like To Have Scabies: Signs And Symptoms
Scabies typically starts with itching and a pimple-like rash, often in areas around your wrists, finger webs, elbows, armpits, waist, knees, ankles, or groin. Sex is a common route of infection.
One of scabies more distinctive effects happens when it worsens.
It keeps people up at night they dont sleep well, and mentally they get very anxious, says Adam Goldstein, MD, MPH, a professor of family medicine at the University of North Carolina School of Medicine in Chapel Hill.
If you think you may have scabies, a trip to your primary care doctor or dermatologist will often resolve the mystery. Under a microscope, a slide with a sample from the irritated area will typically show the mites.
How Does A Doctor Diagnose Scabies
Scabies is usually diagnosed just by looking at the rash, and by the story of itchy spots spreading around your body. A doctor will often be able to recognise the typical appearance of the scabies rash, although in the early stages it can resemble eczema.
Often, a doctor will find one or more mite tunnels on the skin to confirm the diagnosis. Sometimes the doctor will rub some ink on the skin and then wipe it off. If there are burrows in that area of skin, the ink will move along the burrow under the skin. It will be visible as a line after the rest of the ink has been wiped away.
What Are The Symptoms Of Scabies
Itching is the main symptom of scabies and it often gets worse at night. It can take about a month for the symptoms to appear after being infected by the mites.
The itching can affect the whole body apart from the head and neck, although the elderly and infants may develop a rash on these areas. It is common for several people in the same family, and their friends, to become itchy at roughly the same time.

Dyshidrotic Eczema Vs Scabies
Dyshidrotic eczema and scabies both cause a person to develop small blisters on their skin. Dyshidrotic eczema blisters usually appear on the hands and feet, earning it the name ‘hand and foot eczema.’ The blisters tend to clear in two-three weeks, leaving the skin cracked, dry, and red. Although the affected area may be itchy, and tender to the touch, its important to prevent scratching in order to reduce the risk of infection.
Solve The Problem: Scabies Treatment That Works
Primary care doctors can absolutely treat it, but sometimes it doesnt present as straightforward, so you might end up seeing a specialist, says Beth Goldstein, MD, a dermatologist in private practice in Chapel Hill and an adjunct at UNC. Dr. Beth Goldstein and Adam Goldstein, MD, MPH, have coauthored guides for physicians on recognizing and treating scabies.
I have had people with absolutely no rash who are just itchy, says Amy Kassouf, MD, a dermatologist at the Cleveland Clinic in Ohio. Its really an allergic reaction to the mite that makes you so itchy. Not everyone shows that as a rash.
While the itching may be unpleasant, treating it is usually fairly straightforward, as long as you follow the recommended steps. When you go to your doctor for treatment, they may recommend an antihistamine and permethrin, which is applied to the skin to kill the mites. Usually, you apply it at bedtime.
People can itch even if treated, but they will feel progressively better, says Beth Goldstein.
Two applications of permethrin are recommended. Five to seven days after the first treatment, youll need a second treatment with permethrin to kill off mites that have hatched in the interim. This second step is important to prevent having to start the treatment cycle again.

The treatments for eczema and scabies are worlds apart. Since scabies spreads so easily, the primary patient and anyone whos been hanging out with them should immediately seek medical treatment.
In the case of eczema, the patient will gradually develop a regular regime to keep their skin condition in check.
Is There Any Resistance To These Treatments
Treatment failures have been reported with lindane, crotamiton, and benzyl benzoate, and resistance may be emerging to permethrin. Resistance to permethrin is well recognised, but only two cases of ivermectin resistance to scabies have been reported in humans . Resistance can be difficult to determine clinically because treatment failure is usually due to inadequate treatment or reinfestation from untreated contacts.

What About Treating Scabies In Pregnant Women
Permethrin is the treatment of choice for pregnant women. Only a small amount is absorbed through the skin, and this is rapidly detoxified without retention in the body.
Lindane is absorbed in small amounts and can be retained and excreted in breast milk. There are no reports of fetal malformations or abortion after the use of lindane in pregnant women.
Scabies Treatment For Those In Close Contact
It is recommended that once a person is diagnosed with scabies, everyone in their homeas well as their sexual partnersshould also be treated, even if they don’t have symptoms.
Most often, scabies is treated with a skin cream that is applied from the neck down. The most common treatment for scabies is 5% permethrin cream, which is approved for use in people older than 2 months of age. The cream is put on once each night and washed off in the morning. Your healthcare provider might recommend repeating this treatment two weeks later if signs of scabies are still present.
In severe cases, a healthcare provider might also prescribe an oral antiparasitic medication called ivermectin.
Children and babies who have scabies might need a parent to apply the ointment to their face and scalp as well. Talk with your healthcare provider about whether this is necessary for your child.
Even with treatment, it can take up to four weeks for scabies to resolve. In that time, your healthcare provider might recommend the following to control symptoms and make you more comfortable:
- Antibiotics to address any infection
- Antihistamines to reduce itch, particularly at night
- Steroid creams or pramoxine lotion to help control itching

What Does Scabies Look Like
The rash of scabies is a mixture of scratch marks and tiny red spots. Scratched areas may develop crusty sores which can become infected and develop into small pus-filled spots. A widespread rash similar to eczema is very common. The itchy rash can cover much of the body, but the mites are found mainly in the folds of skin between fingers and toes, the palms of the hands, the wrists, ankles and soles of the feet, groins and breasts. The scabies mites burrow into the skin in these areas to lay their eggs, leaving tiny spots and silver coloured lines on the skin. Adult mites are tiny, only about 0.4 mm long which when looking through a magnifying lens or dermatoscope, appear as a tiny dark dot at the end of a silver line.
What Are The Risk Factors
Scabies is transmitted by close personal contact. Infants and children are therefore particularly liable to infection from close physical contact with other children and adults at home and at school. Outbreaks can occur among elderly people in nursing homes and can be transmitted to nursing staff. Transmission between adults is often by sexual contact.

What Are The Signs And Symptoms Of Scabies
After the mite burrows into the skin, it takes time to develop signs and symptoms. If you’ve had scabies before, the itching usually begins within one to four days. When a person has not had scabies, the body needs time to develop a reaction to the mite. It can take two to six weeks to develop symptoms.

Signs and symptoms of scabies include:

Itching, mainly at night: Itching is the most common symptom. The itch can be so intense that it keeps a person awake at night.

Rash: Many people get the scabies rash. This rash causes little bumps that often form a line. The bumps can look like hives, tiny bites, knots under the skin, or pimples. Some people develop scaly patches that look like eczema.

Sores: Scratching the itchy rash can cause sores. An infection can develop in the sores.

Thick crusts on the skin: Crusts form when a person develops a severe type of scabies called crusted scabies. Another name for crusted scabies is Norwegian scabies. With so many mites burrowing in the skin, the rash and itch become severe. You’ll find more information about crusted scabies below.
The severe itch can lead to constant scratching. With non-stop scratching, an infection can develop. Non-stop scratching can even lead to sepsis, a sometimes life-threatening condition that develops when the infection enters the blood.
How Is Scabies Differentiated From Atopic Dermatitis
Scabies manifests in infancy or childhood as a pruritic eruption. Other members of the family are usually itchy, and the primary sites of involvement are moist, warm areas. The eruption is polymorphic with a dermatitis, nodules, urticaria, and 6-10 burrows. Pustules on the hands and feet are almost diagnostic of scabies in infancy. Facial involvement is rare, and xerosis does not occur.

A Pharmacist Can Help With Scabies
Scabies is not usually a serious condition, but it does need to be treated.
A pharmacist will recommend a cream or lotion that you apply over your whole body. It’s important to read the instructions carefully.
Let the pharmacist know if you are breastfeeding or pregnant.
You’ll need to repeat the treatment 1 week later.
Scabies is very infectious, but it can take up to 8 weeks for the rash to appear.
Everyone in your home needs to be treated at the same time, even if they do not have symptoms.
Anyone you have had sexual contact with in the past 8 weeks should also be treated.
Could It Be Something Else
It is sometimes difficult to tell the difference between the rash caused by scabies and the rashes of some other skin conditions. For example, sometimes the itchiness on the hands and wrists is quite similar to eczema. Sometimes doctors will treat the itching with steroid creams, which work well in eczema but will not help scabies at all.
If itching and a rash develop in several people who live in the same home at about the same time then scabies is a likely cause.

Almost all patients with eczema complain of itching. Since the appearance of most types of eczema is similar, elevated plaques of red, bumpy skin, the distribution of the eruption can be of great help in distinguishing one type from another. Getting The Entire Household Scabies Free
In families or households where one person has scabies, youll need to be proactive about treating everyone for scabies even if they dont have symptoms. That goes for babysitters or frequent visitors like grandparents, too.
When one family member in a household gets diagnosed, in order to effectively treat that household, you have to treat everyone who lives there not just the people who have rashes or are itchy, says Gehris.
If you dont successfully treat it, its going to continue, says Dr. Adam Goldstein.
Thats especially important for families to keep in mind. In addition to treating the person with scabies, the bedding, clothes, and towels need to be washed in hot water and dried on a hot setting, or dry-cleaned to rid them of the parasites.
Items that cannot be washed should be sealed in a plastic bag for 72 hours or more to kill the mites. This effectively decontaminates things like toys and other objects handled by the infected person or household members, in order to keep the mites from continuing to spread, notes the Centers for Disease Control and Prevention.

Is Compliance Important
To maximise treatment success, correct application of topical preparations is of cardinal importance. Permethrin should be applied twice, with applications one week apart. Treatment should be applied to the whole body , including web spaces of fingers and toes, the genitalia, and under the nails. In children aged up to 2 years and in elderly and immunocompromised people the application should be extended to the scalp, neck, face, and ears.
Patients should reapply treatment to their hands if they wash them during the treatment period. Mittens or socks may be necessary for the hands of thumb or toe sucking infants and toddlers.
All members of the affected household should be treated at the same time . The application should be washed off after the recommended time and clothes and bed linen machine washed at temperatures above 50°C.

How Do I Diagnose It
A history of itching in several family members over the same period is almost pathognomonic. However, lack of a history of itching in family members does not exclude scabies. It can be notoriously difficult to get family members to admit to a history of possible scabies, and some people with scabies genuinely seem not to itch.
If left untreated, scabies can continue for many months. It is important to remember that recurrence of symptoms after attempted treatment does not exclude the diagnosis of scabies because patients may not have treated themselves correctly or may have been reinfested by an untreated contact.
What Are The Symptoms
Scabies shows itself by severe itching that gets worse at night. The itching happens in thin burrow tracks made of tiny blisters and bumps on the skin found in the folds of the skin.
Symptoms usually take a few days to appear if youâve had scabies exposure before. But if you havenât, it can take up to 6 weeks. However, you can spread scabies even if they havenât yet physically started to show up on your skin.
The rash can appear anywhere on your body, but it commonly appears:
- Between fingers
- Along the insides of the wrists

As for eczema, different types can appear on different parts of the body. They also show up for different triggers. Sometimes, if the itching is too intense, you may bleed. This in turn makes the eczema worse. This is called the âitch-scratch cycle.â
